Job Description


The primary focus is to assist clients with functioning in the least restrictive environment. The case manager is responsible for assessing, linking, monitoring, and advocating for all assigned clients. The case manager assures that all assigned clients received the most appropriate services available to meet their identified needs. Case manager is responsible for any additional duties assigned. To accept the role of being a mentor and a positive leader in support of the Pillars of Excellence.


  1. To provide assessments, Service planning, Aftercare planning, care decisions, and reassessments.

  2. Is compliant with and follow 65-E15, SAMHPO, AOP standards, complete Person to Person training and AHCA regulations.

  3. Completes all documentation and billing of all service provisions with in policy guidelines. And completes daily logs.

  4. Is required to work in office, including field work (working with clients in their home) complete 1-3 home visits weekly.

  5. Must attend all mandatory monthly Departmental, Residential, meetings and Peer supervision.

  6. Attend residential staffing’s.

  7. Adhere to all Agency policy and procedures and review HR manual.

Job Requirements


MINMUM EDUCATION: Bachelor’s in Health Science or related field required; Graduate Degree in Health Science or related field preferred.

MINIMUM WORK EXPERIENCE: At least 3 years of case management experience or Master’s Degree.

REQUIRED SKILLS, KNOWLEDGE AND ABILITIES: To perform this job successfully, an individual must be able to perform the essential duties satisfactorily. Experience in community health preferred.
