Job Description

Job Purpose: As a Case Manager at Aspire Health Partners, your job purpose to provide overall assessment and needs of chronically mental ill adults. These functions include – but are not limited to – providing screening, referral, emergency, crisis and case management services, participating in clinical treatment team, documenting clinical records and additional program specific documents while maintaining the integrity of the client, understanding and ability of the necessary technology to communicate effectively with staff and, on occasion, act as the authorized client transporter for necessary treatment and services. Most importantly, all functions must be completed while maintaining high standards of ethical and professional conduct while adhering to agency policies and procedures.

  1. Demonstrate competency in the knowledge of community resources.
  2. Participate in staffing’s and other interagency meetings.
  3. Assist staff and clients in preparing accurate assessments of their housing needs.
  4. Assist staff and clients in obtaining supports support necessary to maintain housing in the community.
  5. Assist staff and clients with presentations of skill training to assist in the development of daily living skills, as needed.
  6. Develop and maintain networking relationships with existing supports in the community to help assure housing permanency.
  7. Develop new community housing resources.
  8. Provide crisis intervention and support as needed.
  9. Enhance job performance through continued training and staff development.
  10. Adhere to all agency policies and procedures.
  11. Perform other duties as required.
  12. Provide service coordination for assigned caseload (case management)

Job Requirements

Position qualifications: Ability to receive and maintain a(n)

  • Level II Background clearance
  • Acceptable Motor Vehicle Registration driver’s license record in accordance with the underwriting guidelines set by the Aspire insurance company as described in the AHP Corporate Transportation Policy 3.9.1


  • Bachelor Degree in a Human Services field

Other qualifications: BA/BS degree in social work, psychology, or Human services related field.   Experience in community health preferred.
