Job Description

Job Purpose:

As a Case Manager at Aspire Health Partners, your job purpose is to participate as part of interdisciplinary team that provides services to individuals with chronic mental illness and/or substance abuse diagnoses.

These functions include:

  1. but are not limited to – providing screening, referral, emergency, crisis and case management services, participating in clinical treatment team, documenting clinical records and additional program specific documents while maintaining the integrity of the client, understanding and ability to utilize the necessary technology, including the electronic record systems, to communicate effectively with staff and, on occasion, may include client transporting for necessary treatment and services and/or coordination of services to meet client needs.
  2. Additional duties assigned may include administrative tasks, verification of benefits, obtaining authorizations, drug screenings, processing of labs, data collection, and assistance in maintaining patient charts. Most importantly, all functions must be completed while maintaining high standards of ethical and professional conduct while adhering to agency policies and procedures. Other duties to be assigned at the discretion of management to best meet the needs of the program.

Job Requirements

Position qualifications: Ability to receive and maintain a(n)

  • Level II Background clearance
  • Acceptable Motor Vehicle Registration driver’s license record in accordance with the underwriting guidelines set by the Aspire insurance company as described in the AHP Corporate Transportation Policy 3.9.1

High School Diploma required, Bachelor degree preferred

Other qualifications: Experience with outpatient services for substance abuse or community health preferred.
