Job Description

Job Purpose: As Peer Mentor at Aspire Health Partners, your job purpose is to provide outreach in order to engage in services to increase self-sufficiency of clients and their families. These functions include – but are not limited to – linking clients and their families to Case Management, conducting screening assessments, assisting clients with obtaining documentation such as replacing ID, birth certificates and social security cards, collaborating with Case Managers to support the clients achieving their goals, completing required outreach forms and maintaining required records, servicing as a peer mentor, adhering to all contractual obligations of the program, serving as a representative to designated committees for community planning and development, completing required training, completing required paperwork, utilizing required technology to efficiently complete tasks and communicating effectively in order to de-escalate high stress situations. All functions must be completed while maintaining high standards of ethical and professional conduct while adhering to agency policies and procedures.

Job Requirements

Position qualifications: Ability to receive and maintain a(n)

  • Level II Background clearance
  • Acceptable Motor Vehicle Registration driver’s license record in accordance with the underwriting guidelines set by Aspire insurance company as described in the AHP Corporate Transportation Policy 3.9.1


  • Associate Degree in a human or related services field
